Uniting Care, Enhancing Lives: Bridging Disability and Aged Care for a Better Tomorrow

Why a Partnership?

The NDISLINK and Health2Ageducate brands bring together the strengths of both the NDIS and Aged Care sectors, creating a powerful synergy that enhances care delivery and support services across the lifespan.

Both sectors share a focus on personalized, person-centered care, driven by a commitment to improving the quality of life for individuals with unique and often complex needs. With government funding, regulation, and workforce challenges affecting both, our organization bridges the gap between these two vital areas of support.

By connecting NDISLINK and Health2Ageducate, we foster a platform for cross-sector collaboration, enabling providers to share innovations, best practices, and resources.

This alignment ensures smoother transitions for individuals moving from NDIS support into Aged Care, while also creating more efficient and integrated service delivery models for providers.

Our approach emphasizes building a continuum of care, where providers from both sectors can work together to address the evolving needs of clients.

With a focus on knowledge sharing, operational efficiency, and interdisciplinary collaboration, we are shaping a unified network that strengthens the connections between disability and aged care services.

Through these efforts, we help providers deliver holistic, high-quality care, driving better outcomes for the individuals they support.

NDISLINK and Health2Ageducate are dedicated to empowering the sectors to work together, advancing innovation, improving service quality, and enhancing lives across Australia.
